The Family Medical Center, located within the Pinckneyville Community Hospital, is here to treat you and your family.  From routine preventative care to chronic health conditions and minor emergencies, our dedicated healthcare providers are here to ensure that you receive the very best care and advice available. 

 

Services include Family Care, Pediatric Care, Internal Medicine, General Surgery, Laboratory Testing, Physical Exams, Immunizations, Care Coordination, Pain Management, Women’s Health and Medicare Wellness Visits.

 

FMC provides Care Coordination to maximize the value of care being delivered and ensure that the patient’s needs and preferences are known and addressed.  The goal is to provide safer and more affective care by sharing pertinent information among all of the providers involved with the patient.

 

Chronic Care Management

 

We provide Chronic Care Management and Medicare Wellness visits to keep you healthy, minimize costs and provide more efficient care.  You may contact the Care Coordinator at 618-357-8820 to learn if this program is right for you.  Chronic Care Management consists of having your personal care coordinator work with you to identify problems related to your chronic conditions.  You will develop a Plan of Care together and take steps to reach your goals.  Your Care Coordinator will call you monthly (at a minimum and more often if needed).  The Care Coordinator will also receive your Medicare Wellness Visit information to review your preventative care. 

 

Medicare Wellness Visits

 

Medicare Wellness Visits are an annual preventative care visit covered by Medicare Part B which means the patient pays nothing for this visit.  The nurse reviews your medical and preventative health history prior to a complete review of your personalized health plan with your provider.  The goal is to help you stay healthy and benefit from information from this visit.  

 

Family Medical Center Hours

 

Monday - Friday:  8:00 a.m. - 5:00 p.m.

Saturday: 8:00 a.m. – 4:00 p.m.

Sunday: 12:00 p.m. – 4:00 p.m.


The Family Medical Center will be closed on New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas. Should the holiday fall on a weekend, the clinic will be closed on both the holiday and the closest weekday. For example, if Christmas Day falls on a Saturday, FMC will be closed on Friday and Saturday. If the holiday falls on a Sunday, FMC will be closed on Sunday and Monday. Weekend Walk-In Clinic Hours

 

Injuries and illnesses don't always wait for a weekday appointment.  The Weekend Walk-In Clinic at the Family Medical Center is available on Saturday and Sunday with no appointment necessary.  Not a regular patient, not a problem.  The clinic is open to the public from infant to geriatric.  

 

Saturday: 8:00 a.m. – 4:00 p.m.

Sunday: 12:00 p.m. – 4:00 p.m.

 

Same-Day Appointments

 

It is our goal to provide you with high-quality, convenient medical care by a carrying staff. There are times, however, that it may be difficult to get an appointment with your personal family physician on the same day as you call the clinic to schedule an appointment. If you have a medical problem you believe requires a same-day appointment, we encourage you to call as early as possible during our clinic hours to schedule an appointment with your primary provider. If your primary provider does not have an available appointment time, we encourage you to schedule an appointment with any of our providers that have an available appointment.
 

Cancellation of Appointments

 

We ask that you provide at least 24 hours advance notice to cancel any appointment. If however, due to circumstances not under your control, you are unable to keep your appointment, please let us know as soon as possible so that we may schedule another person requiring care.
 

Prescription Refills

 

Please call your pharmacy regarding refills on medication at least 72 hours in advance, to allow sufficient time for the pharmacy and your provider to receive and respond to your request before you run out of your medication.
