Pinckneyville Community Hospital’s Family Medical Center Received Patient Centered Medical Home (PCMH) Accreditation

 

 

The Pinckneyville Community Hospital is proud to announce that the Family Medical Center has been reaccredited by The Compliance Team as a Patient Centered Medical Home (PCMH) and as a Rural Health Clinic (RHC).  The Compliance Team is a nationally recognized healthcare accreditation organization which provides comprehensive accreditation and certification services to healthcare providers. 

 

Patient Centered Medical Home (PCMH) Accreditation is a measured Continuous Quality Improvement (CQI) program that incorporates patient-centered practice guidelines. Patient-centered programs and value-based care are advanced by the Institute of Medicine and the nation’s leading primary care physician societies. The Compliance Team’s plain-language Safety-Honesty-Caring Quality® standards focus on day-to-day practice operations. Special emphasis is given to health maintenance, preventative screening, multi-specialty medical services, patient experience reporting, and bench-marking.  High-risk patients benefit from PCMH’s pro-active coordination and follow-up communication by Physicians, Physician Assistants and Nurse Practitioner’s. Having their patients ongoing needs addressed prevents trips to the hospital and saves the patient time and helps to reduce unscheduled health emergencies. 

 

The Rural Health Clinic (RHC) Accreditation program is intended to increase access to primary care services for patients in rural communities. To receive certification, the Family Medical Center was required to use a team based approach of physicians working with non-physician providers, such as nurse practitioners (NP) and physician assistants (PA), to provide outpatient primary care services and basic laboratory services.

 

Along with the PCMH accreditation and RHC accreditation, the Family Medical Center was also recently recognized for providing services above the National ratings of all Rural Health Clinics in several categories which were:  Getting Timely appointments, Helpful, Courteous & Respectful Office Staff and Provider Ratings. 

 

Recently, the Family Medical Center opened in their new 19,000 sq. ft. building to better serve and meet the medical needs of the area residents.  The Medical Staff is composed of Dr. Christopher Reyes, Dr. Andrew Forbes, Dr. Sabrina Haque, Dr. Marie Gorski, Steve Priebe, PA-C, Angie Eubanks, PA-C, Amber Kellerman, FNP-BC and Miles Priebe, FNP-BC.
